The auxiliary cable fault tester is connected to multiple devices and devices. Before dismantling scrapped cables, it is necessary to carefully inspect the drawings and real objects. Corresponding voltage and current meters should be used to measure the required dismantling of i-coated cables. If cable dismantling involves public circuits, bonding measures must be taken to ensure that the cable does not affect the operating equipment before dismantling. The following are incorrect disassembly or cable disassembly areas that should not be noted. The situation that caused the accident.
No safety isolation measures were taken for live equipment to verify the 35kV self switching and false tripping of the main transformer.
(l) Cause of the accident:
All self cutting protection functions have been verified. The cable removed from the terminal block of the self cutting protection screen to the main transformer protection screen was not wrapped, and during operation, the main transformer trip line in the cable collided with the positive power line, resulting in misoperation of the main transformer.
(2) Accident Summary:
1) Work can only begin after holding on-site shift meetings. The person in charge should inform each shift worker of the content and danger points of the work ticket, explain safety and technical measures, and confirm that each shift worker is aware of them.
2) Issue safety measures tickets, check whether the on-site safety measures meet the requirements, and correctly complete the isolation measures between operating equipment and maintenance equipment (including before and after equipment shielding).
3) Do not use experience instead of equipment drawings, and use working memory instead of safety measures. Before starting work, you must read the drawings clearly. Without drawings, work is not allowed.
4) It is necessary to record the removal of secondary fine wires, isolate live parts, perform strict calibration during restoration, and conduct necessary group testing to verify the correctness of the circuit. After the entire group testing, secondary fine wires should not be removed.
